After Care Program

M.A. Clark & Sons Funeral Home is there for you and your loved one before, during, and after the loss.  After Care is a funeral service follow up consultation that we provide following the funeral or memorial service.  Its purpose is to help survivors, next of kin, or executors to resolve some of the estate matters that are imperative after the loss of a loved one. 

Items that will be covered include:



  • Canadian Pension Benefits
  • OAS Benefits
  • SIN Number and card
  • Health Card
  • Birth Certificate
  • Passport


Retirement and Pensions

  • Life Insurance Policies, annuities, etc.


Canada Pension Plan and Old Age Security

  • Application for lump sum death benefit
  • Application for survivors benefit and children’s benefits (if applicable)


Credit Card Cancellations

  • We will contact companies as needed for you



  • Application for life insurance benefits
  • Updating beneficiaries on existing policies
  • Continuance of Health Insurance


Bank Account Updates

  • Adding names of children or other sunnrvivors
  • Bank procedures on closing accounts
  • Joint account information


Documents to bring with you during After Care (not to worry if not all are found):

  • Will- with signatures OR notarized copy
  • Birth Certificate
  • Health Card
  • Social Insurance Card
  • Passport
  • Driver’s License
  • Life Insurance Policies/Info
  • Employment Benefits information
  • Credit Cards
  • Charge Account Cards
  • Your personal identification
  • Veteran’s Affairs ID (If applicable)
  • List of contacts for any Associations
  • VOID cheque for joint bank account
